John Nelson, MD, professor of pediatrics at the University of Texas–Southwestern, Dallas, has noted that the article by Principi et al in the December 1989 issue of AJDC1 failed to include a note concerning the use of trimethoprim-sulfamethoxazole as prophylaxis for otitis media. The thrust of Dr Nelson's comments was the fact that the manufacturer of the medication clearly indicates that "the drug is not indicated for prophylactic or prolonged administration in otitis media at any age." He further indicated that Principi et al should have noted this statement in the report. International correspondence has resulted in confusion on the central issue and I thought it best to summarize the discussion in this editorial note.
