A 13-year-old girl came to the endocrinology clinic with the complaint that she had not grown in height during the past three years. She had gained weight progressively during this time despite attempts to diet, and was disturbed by dry skin and easy fatigability. There was no history of constipation or cold intolerance. She also complained of pain in the left hip that was of three months' duration, and her father noted that she had walked with a limp for several months. The patient was a short, obese, slow-moving girl with periorbital puffiness and a masklike affect. Her thyroid gland was not palpable. She had a full range of motion in her hips but walked with a limp. Her skin was dry and scaling and her reflexes were dull. Her thyroxine level was 0.7 µg/dL (normal, 5.0 to 13.7 µg/dL her thyrotropin level was 432 µU/mL (normal, <15 µU/mL), and
